Why is a health check-up a good idea?

You can get advice about improving your health and well-being. It can also catch the onset of some diseases early. E.g. Diabetes, high blood pressure, heart problems, asthma etc.

Health Assessments for over 75s

If you are over 75 years of age you are eligible for a FREE Health Assessment once per year.

This health assessment help solve problems before they happen.

It also identifies things that could be done to help you live independently in your own home for as long as possible.

Are you in the 45-49 age group?

A number of risk factors could lead to a chronic disease later in life. We have a specially designed health check for people in this age group. This health check will be Bulk Billed.

ATSI Health Check Closing the Gap

Do you identify as Aboriginal or Torres Strait Islander?

Register for Closing the Gap. Your Doctor can then prescribe PBS medicines that will be more affordable.

You will be eligible for a free Health check each year and access to additional support services for ‘at risk’ patients.

Mole and Skin Checks

Come and talk to one of our doctors if you notice a freckle, mole or lump that is new or changing in size, shape or colour, or a sore that does not heal.

Childhood Immunisations

Immunisation from an early age is highly recommended. Having a child immunised helps protect them against most serious childhood infections, some of which may be life-threatening.

Chronic Disease Management

Management Plans

A GP Management Plan involves your GP developing a written plan of management in consultation with you.

The GP Management Plan is a written set of information about what you need in the management of your chronic and complex condition.

Team Care Arrangements

After preparing a GP management Plan, your GP may identify that you could benefit from the input of other health providers. Your GP may recommend a Team Care Arrangement. This involves at least two other health professionals in addition to your GP. With your consent, your GP will ask the relevant health providers to form a team and work together in developing a plan based on your care requirements. The Practice Nurse may also assist in this process.

Who will benefit from a Management Plan?

People with a chronic or terminal medical condition that has been or will be present for 6 months or longer. For example those with Asthma, Diabetes or Heart Disease

Preparing for a GP Management Plan.

When your GP decides to prepare a GP Management Plan the following will occur.

Together you and your GP decide:

  • What your health problems and needs are
  • What goals you would like from the plan
  • What, if any other health care services you need.

In some cases the Practice Nurse may assist in this process. You may also wish to have your carer or a family member present when preparing your plan. When preparing a plan your GP will consider your physical, psychological and social needs.

How often can a GP Management Plan be done?

GP Management plans can be prepared annually and the reviewing is just as important as preparing the plan. Plans should be reviewed every 3 months. Earlier reviews can be arranged if your condition changes significantly.

These services only include “private” providers, not services delivered from within a public hospital.

Are there Medicare Rebates?

People with a GP Management Plan and a Team Care Arrangement are eligible for 5 Medicare rebates per year for services provided by “private” health care professionals. The health care professional must be registered with Medicare before Medicare will rebate the service. Please note there may be a gap payment. Check with the health care professional before your appointment.

Important Facts about Management Plans
  • Your GP will decide if you fulfill the eligibility requirements set by Medicare Australia prior to preparing the plan.
  • The time required to prepare a plan will vary depending on how complex the condition is.
  • People with Team Care Arrangements can access a total of five (5) health care professional services a year.
  • Health care services under Team Care Arrangements are not free. A rebate is provided by medicare but there may be gap payments.
  • Each person with a GP Management Plan will be offered a copy of the plan.
  • Maintaining and improving standards of General Practice.
  • Bringing General Practioners closer to the community.
  • Identifying the health care needs of the community within its boundaries and facilitating programs and services to meet those needs
  • Providing educational and practical support for the optimal delivery of primary health care in the Division area.
  • Facilitating links between General Practitioners and other health care professionals throughout the Division.
  • Promoting research programs which focus on issues relating to rural General Practice.

Fees and Billing

Slade Point Medical Centre is a mixed billing practice. To be eligible to be Bulk Billed you will need to show proof of being on a Centrelink Health Care Card or Pension Card.

All patients under 16 years old are bulk billed.

Patients are allocated 10 minutes for an appointment, if you feel you will require longer than 10 minutes, please let the reception staff know when booking your appointment. When booking in for the whole family, individual appointments are to be made for each family member who is planning on seeing the GP.

General consultations Monday – Friday and Saturday until 1pm:

Standard Consultation – $95
Long Consultation – $125

A Medicare Rebate is available straight away for these consultations.

After 1pm on a Saturday and all day Sunday:

Standard Consultation $115
Long Consultation $135

A Medicare Rebate is available straight away for these consultations.

Please be aware that the Medicare Rebate needs to go onto your Savings or Cheque account which is linked to your Medicare card.

Whilst we are a Mixed Billing Practice there are some items that are not covered under the Medical Benefits Scheme such as Private and Commercial Drivers Licence, minor procedures or certain vaccinations. You will be advised of any fees in advance. Your Medicare Card will be validated online at each visit.
